The scholars will tell you: ,and then.
Some of the best and most successful writers fequently use:, then. In many cases it packs more punch.
As the greatest suspense writer of them all said, "Never let grammar stand in the way of a good sentence." He was a graduate of an Ivy League university.

I use either of them, depending upon what I am saying and how I want it to come across.

Ex: Suddenly, there was a loud noise. Then, as if from nowhere, he came charging at me....

My take is if you can eliminate "then" don't use it!..."Then" slows action..Many words like that don't improve the writing.

Better Ex: Suddenly, there was a loud noise. He came charging at me!

Get it?...De Gustibus!...Rudy
